I believe that it is a good practice to do code review as often as possible. To make code review process smooth, productive and painless developers need a good tool. Since I use Mantis (http://www.mantisbt.org/) in my projects - I would love to have code review feature for mantis too. 

There are few code review tools available already, but no one integrates with mantis: 

Code Collaborator (http://www.smartbear.com/codecollab.php) &#8212; Not free (expensive). Many cool features. Root access needed for installation. Written in PHP. Review Board (http://www.review-board.org/) - Open Source solution. Very basic functionality. Not very logic in some cases. Written in Python. Acunote (http://www.acunote.com/promo). Not free (expensive). It is agile software development system. Watch this screencast: http://www.acunote.com/promo/screencast This part is especially interesting for us: http://www.acunote.com/tour/code_inspection What could we do to fix it? A plugin! Simple (or not so simple) plugin that provides option to plan and do code reviewing. Of course, integration with SVN is needed.
