The Patient Registry project aims to deliver an information system with the features needed to support improved outcomes for chronically ill patients and to track the overall health of a population. The target audiences are chronic disease care providers, case managers, clinic staff, physicians, multi-disciplinary care teams, healthcare resources decision makers, public health officials, epidemiologists and anyone that needs help in working with chronic disease patients or those at risk.
